When creating a form, there are many different types of fields that you can choose from. One decision that needs to be made is whether to use checkboxes, radio buttons or drop down lists? Checkboxes are best when you have multiple choices and can select two or more of the choices. Radio buttons are best when you have multiple choices but only one answer is correct (e.g., yes, no; hot, warm, cold). Drop down lists are best when you have many items in a list but can only select one item.
