When to use Checkboxes, Radio Buttons and Drop-Down Lists

There are three types of fields (checkboxes, radio buttons and drop-down lists) that have similar functionality so when do you use one field type over the other?

Checkboxes should be used primarily when you want to present many choices and want to allow the person filling out the form to be able to select two or more of the choices.

Radio buttons are the right option when you want to present two or more choices but the choices are mutually exclusive (Yes/No) meaning that the person filling out the form can only select one choice.

Drop-down lists should be used when you have lots of choices to select from and are limited in how much space is available for the placement of this field on the form. The person filling out the form can view the list of choices but can only select one item.

In the form below, the person who designed this form used all three field types.
