Some forms such as surveys and questionnaires often ask questions that provide you with some type of a scale or ranking where you can respond along a continuum. There are various ways to build these type of forms. One way is via a radio button where you create the scale (i.e., 1 through 10). Another way is where you create text labels above and below checkbox fields. Both approaches are displayed below.
