Most government agencies and some corporations have a work permit process. It usually entails requiring an individual or a contractor to fill out a form stating what they are seeking permission to do.
The different types of permits are too great to cover in this blog post but a sample form that was created using the FormConnect Pro app is shown below. What I like about this form is that they separated the various sections with black horizontal and vertical lines and red text to make the field labels stand out.
