A doctor in South Africa is using the FormConnect app for documenting what occurred during the surgery he performed. The information includes the patient name, the names of the other doctors that participated, the starting and ending time and the procedures performed.
There is nothing unique about the information being collected. What is unique is how a doctor replaced the paper form that he previously used with an iPad and the FormConnect app to perform this task.
