One of our customers has created a multi-page pre employment medical questionnaire. It covers various areas of a person’s overall health that would occur during a regular medical examination. One area that is covered is the testing of the employee’s vision. Below is one page of this questionnaire where the eye doctor fills out the form and documents the applicant’s visual acuity.
