The iPad is the perfect device for filling out a form or a questionnaire. It’s form factor is similar to a clipboard and can be cradled in one hand while you are sitting or standing. Below is a sample questionnaire that one of our customers created that allowed them to hand the iPad to their patient to fill out. The questionnaire is very intuitive and the radio buttons on the form can be engaged by just tapping on the button.
