One of the keys to designing forms is to align the field boxes (text, drop-down, radio buttons, check-boxes) etc. If you drag these fields below each other, they will all align with each other. Another tip is to hide the text. This can be done by un-ticking the “display label” setting so that the field name does not display. The field setting has an option to hide the text.
