A wholesaler in the picture framing industry uses the FormConnect app for documenting any goods that are returned. The form below shows the information that they collect. Besides the customer name and the product being returned, there is also a field called “reason code” where the customer can choose a value from 1 to 6. These reason codes are displayed toward the bottom of the form in a legend describing what each code means.
