Scoring with Dropdown Lists

A dropdown list was used to evaluate performance in the design of the form below. The criteria used to rate performance was based on a scoring system 0 through 5 with 0 = “Failed to perform the job requirements” and 5 = “Superior performance of the job requirements.” To create this scoring system, a dropdown list was created with the five values plus NA = “Not observed”. Then, a legend was created that is displayed on the form describing what these six scores mean.
